Request Copy of Police Report

A request for a copy of a police report can be done by completing this form. If you are a law enforcement or county agency kindly submit your request on letterhead via email at policerecords3@losaltosca.gov or fax to (650) 947-2704.

Due to the sensitivity of some reports, not all reports are releasable; even to involved parties. If you are unsure of any of the details when making this request, fill out the form as best you can and someone from Records will contact you and be available to assist you throughout the process. Case Number (Format: 00-0000) should be noted on the reverse of the Officer's business card that was given to you. If you don't know your case number, please complete this form with as much information as possible.

Juvenile Reports

All Juvenile report requests must be approved for release by the presiding Juvenile Court Judge. If your request for report involves a juvenile, submit this form along with JV-575 & JV-580. Los Altos Police Department staff can assist you with this process.
